Health Office Rules

  • Except in an emergency, all students in Grades 5-12 will have a signed pass from their classroom teacher. Do not arrive between classes.

  • If the nurses are not in the office, return to class! Do not wait in the nurse's office anticipating their return. It is extremely rare that the health office is vacant.

  • It is against school policy for students to make arrangements for early dismissal due to illness without being evaluated by a school nurse. Do not use the classroom phones, or cell phones to call home because of illness.

  • If a student is being excused due to illness, the student is NOT allow to drive home and transportation must be provided.

  • Students requiring medication during school hours must first have on file a written request form signed by both the parent and the prescribing physician. Forms are available at the nurse's office. NO MEDICATION (PRESCRIPTION OR OVER-THE-COUNTER) WILL BE ADMINISTERED WITHOUT THIS PERMISSION FORM ON FILE. Students are also reminded that all medication is to be kept in the nurse's office unless instructed otherwise by the health office.

Medications in Schools

If your child needs to receive medication at school or needs to self-carry medication to sporting events:

  • The school nurse must have a written order from the Medical Provider (MD, PA, NP) in which he indicates the name of the medication, dosage, route of delivery and time(s) medication is to be given.

  • The school nurse must have a written parental permission slip on file which allows the nursing staff to give the order medication(s).

  • All medication delivered to the school are to be in a container provided by hour pharmacy. The label is for identification only and does not constitute a written order for the Medical Provider.

  • Medication should be delivered directly to school by the parent/guardian.

  • Students are reminded that all medication is to be kept in the nurse's office unless the student has a self-carry order from their Medical Provider and parental permission slip on file in the Health Office.

  • A new order is required for each school year.

Mandatory Screenings

The district's School Health Services program supports your student's academic success by promoting health in the school setting. One way that we provide care for your student is by performing the health screenings as mandated by the State of New York. During the school year, the following screenings will be required or completed at school:

Vision

  • Distance and Near vision acuity for all newly entering students and students in PK or K, Grades 1, 3, 5, 7, and 11.

  • Color perception screening for all newly entering students.

Hearing

  • Hearing screening for all newly entering students and students in PK or K, Grades 1, 3, 5, 7, and 11.

Scoliosis

  • Scoliosis (spinal curvature) screening for boys in grade 9 and girls in grades 5 and 7.

Health Appraisals

  • A physical examination including Body Mass Index and Weight Status Category Information is required for all newly entering students and students in Pre-Kindergarten or Kindergarten, Grades 1, 3, 5, 7, 9, and 11.

Dental Certificates

  • A dental certificate is requested for all newly entering students and students in PK or K, Grades 1, 3, 5, 7, 9, and 11.

A letter will be sent home if there are any findings on the screening done at school that would cause concern or need medical follow-up.

Sports Physical News

Physicals must be completed before students can participate in any athletic practice.. Sport physicals will be offered during the school year. In addition to physicals, if the student wishes to play a subsequent season, Interval Health History forms must be completed. For example, if a student played soccer in the fall and wants to try out for basketball in the winter, he/she must complete an Interval Health History before he/she will be eligible to practice. To be active in sports at BMC, each student will need to complete his/her paperwork and physical for each athletic season.

Body Mass Index

As part of a required school health examination, a student is weighed and his/her height is measured. These numbers are used to figure the student's body mass index or "BMI". The BMI helps the doctor or nurse know if the student's weight is in a healthy range or if it is too high or low. Recent changes to the New York State Education Law require that BMI and weight status group be included as part of the student's school health examination. Furthermore, a sample of school districts will be selected to take part in a survey by the New York State Department of Health. If our school is selected to be part of the survey, we will be reporting to New York State Department of Health information about our students' weight status groups. Only summary information is sent. No names or information about individual students will be submitted. However, you may choose to have your child's information excluded from this survey report. The information sent to the New York State Department of Health will help health officials develop programs that make it easier for children to be healthier. If you do not wish to have your child's weight status group information included as part of the Health Department survey this year, please complete the BMI opt-out form provided in the school nurse download section and submit it to the Health Office. Public Notice:

Student Accident Insurance Coverage: Brushton-Moira Central District students and athletes are covered by an excess insurance policy. All injuries that require medical attention must be reported to the school' s health office as soon as possible.
